chore: add .gitignore and clean up repository

- Added comprehensive .gitignore for node_modules, dist, env files
- Removed node_modules from git history (keep locally)
- Removed empty/placeholder files (EOF, PLANEOF, create-staging.py)
- Repository now clean for public sharing
